CDC19-033 Notice is hereby given that quotes will be received by the Community Development Commission of the County of Los Angeles (Commission) until Thursday, February 21, 2019, at 3:00 pm for Single Family Home Improvement Program. This will include furnishing all labor, material and equipment, and performing all work necessary and incidental in accordance with the Scope of Work (SOW) and contract documents. Bidders must attend a mandatory site visit at the following date, time and location: Wednesday, February 13, 2019, at 12:00 PM Homeowner Name: Raymundo and Carmen Martinez Street Address: 1460 N Siesta Avenue City, State, ZIP: La Puente, CA 91746 Telephone: (626) 419-4012 A Request for Quotes (RFQ) solicitation package containing all submission requirements may be downloaded from the Commission’s website at www.lacdc.org, under “For Vendors – View Open Solicitations”. Bidders are advised that this RFQ is an informal solicitation, and should not be construed as engaging in a formal competitive bidding pursuant to any statute, ordinance, rule or regulation. Bidders shall provide their quote, all required forms, and documentation by the due date and time stated herein, and shall deliver it via regular mail or in person to: Maria Ortiz, Procurement Coordinator Community Development Commission of the County of Los Angeles Housing Investment and Finance 700 W. Main St. Alhambra, CA 91801 All quotes shall be labeled “Quote for Single Family Home Improvement Program RFQ No. CDC19-033, 2/21/2019”. Any Bidder who wishes his/her bid to be considered is responsible for making certain that it is received by the Commission at the stated location and at the stated date and time. No oral, telephonic proposals, or modifications will be considered unless specified. Quotes received after the scheduled deadline will be returned unopened. In submitting a quote, each Bidder thereby agrees that, if awarded a contract, Bidder shall execute a standard agreement, a sample of which is attached to the RFQ package for reference. The Commission shall not be obligated to facilitate an agreement between the homeowner and the contractor with any Bidder who refuses to execute said standard agreement. The Commission reserves the right to reject any and all quotes. This RFQ is not a contract or commitment of any kind. The Commission is not liable for costs incurred in the preparation of the respondent’s quote. It reserves the right to issue supplementary information or guidelines related to this RFQ. REQUIREMENTS: • Insurances ? General Liability with Additional Insured Endorsement, shall include Completed Operations and be on a primary and non-contributory basis. ($2M aggregate; $1M each occurrence) ? Workers’ Compensation, shall be Statutory and include Employer’s Liability, and a waiver of subrogation. ($1M each accident) ? Automobile Insurance, coverage shall include owned, hired, non-owned, OR any auto. ($1M each incident) ? Pollution with Additional Insured Endorsement, shall include Completed Operations and be on a primary and non-contributory basis. ($500K aggregate; $250K each occurrence) ? Crime or CRIME BOND or FIDELITY BOND with Additional Insured Endorsement, must name the Commission as Loss Payee OR provide copy of “Client Coverage Endorsement” – shall cover EMPLOYEE theft and/or forgery. ($50K each occurrence) • Evidence of a valid B-1 General Contractor’s license. • EPA Certification. • (If applicable) SCE/SoCalGas® Energy Upgrade California Participating Contractor.
