Description
1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued.
1.A. Project Title: Brand Name or Equal to JLG 1055 Telehandler 10,000 pound, Variable Reach, Rough Terrain Forklift/Telehandler.
1.B. Project Location: West Los Angeles VA Medical Center, DELIVER TO VHAGLA WAREHOUSE BLDG 297, 11301 Wilshire Boulevard, Los Angeles, CA 90073-1003.
1.C. Solicitation Type and Number: Request for Quotes (RFQ) - 36C26223Q0320.
1.D. Set Aside: This solicitation is Set Aside for Service Disabled Veteran Owned Small Businesses (SDVOSB).
1.E. Applicable NAICS code: 333924, Industrial Truck, Tractor, Trailer and Stacker Machinery Manufacturing.
1.F. Small Business Size Standard: 750 Employees.
1.G. Type of Contract: Firm Fixed Price.
1.H. Period of Performance: 30 days from date of award.

2. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit the Standard Form 1449 pages 1, 3 and 4 via email no later than January 03, 2023 at 1:00 pm PT to theodore.arlauskas@va.gov.
