San Joaquin County Human Services Agency (HSA) is soliciting proposals fromqualified agencies to provide Transitional Housing Program Plus (THP-Plus) services foremancipated foster/probation youth. THP-Plus was established by the California State Legislature (Assembly, Bill 427) in2001 to provide housing and case management for youth emancipating from foster orjuvenile probation placement. Emancipated youth are those who have been in courtordered foster or juvenile probation placement between the ages of 16 and 18-years old. THP Plus services include, but are not limited to: •Case management. •Crisis intervention support. •Therapy and medical care referrals, as needed. •Educational advocacy and support. •Job readiness training. •Assistance in finding or maintaining affordable housing. •Assistance in reaching Transitional Independent Living Plan goals. THP Plus services will be funded for the period July 1, 2023, through June 30, 2025, with an option to extend up to two (2) tears. HSA currently contracts out for 24 slots per month. The number of slots per month for future fiscal years will be dependent on available State and Federal funding. Total funding each fiscal year shall not exceed the State allocation to the County for the purposes of the services contained in this RFP. The allocation for Fiscal Year 2023-2024 is $1,121,043. A single contract, or multiple contracts, may be awarded to the proposer(s) whose submission is determined to be most responsive to this RFP.
